Das Hauptwerk des Boethius ist die Consolatio philosophiae („Trost der Philosophie") in fünf Büchern. Außer diesem Titel kommt auch die Form De consolatione philosophiae („Über den Trost der Philosophie") in den Handschriften vor. Das Werk entstand nach Boethius' Verhaftung. Mit der Gestaltung als Prosimetrum (Prosa mit eingefügten Gedichten) greift Boethius eine in der Spätantike beliebte Form auf. Er verwendet 28 verschiedene Versmaße. Die Consolatio philosophiae besteht aus 39 Prosatexten und 39 Gedichten, die abwechselnd aufeinander folgen. Das dargelegte philosophische Gedankengut stammt vor allem aus den Werken Platons, des Aristoteles und der Neuplatoniker, aber auch stoische Vorstellungen sind eingeflossen. Auf die Lehren Platons wird immer wieder zustimmend Bezug genommen. Das Werk reiht sich in die Tradition der antiken Trostliteratur ein und ist zugleich ein Protreptikos (eine zur Philosophie ermunternde Schrift). Dargestellt wird der Heilungsprozess des in seiner Not seelisch erkrankten Gefangenen. Das Werk zerfällt in zwei Hälften, wobei das berühmte, ungefähr in die Mitte gestellte neunte Gedicht des dritten Buches (Anfang: O qui perpetua) den Übergang und Wendepunkt bildet. Im ersten, negativen Teil wird dem Leser die Nichtigkeit der irdischen Güter und die Sinnlosigkeit des Strebens nach ihnen vor Augen gestellt. Im zweiten, positiven Teil richtet sich das Augenmerk auf die Alternative zu diesen vergeblichen Bemühungen: die zum Erfolg führende Suche nach dem einzig wahren Gut, dem Guten schlechthin. Ob der abrupt wirkende Schluss stimmig und das Werk somit als abgeschlossen zu betrachten ist, ist in der Forschung umstritten. Unklar ist auch, ob die Schrift schon vor dem Abschluss des Gerichtsverfahrens oder erst nach der Verhängung des Todesurteils entstanden ist und ob Boethius sich im Kerker oder in einem relativ komfortablen Hausarrest mit Bibliothekszugang befand.
