In the years since the initial discovery that blood from diabetic patients contains increased amounts of a posttranslationally gluco sylated form of hemoglobin (hemoglobin Ale)' an impressive number of studies have clarified and expanded the use of glycohemoglobin levels to assess disease status. Many other structural proteins have been shown to undergo similar changes, including proteins from tissues most commonly affected in diabetes (e.g., lens, aorta, peripheral nerve, basement membrane). Thus, the nonenzymatic glycosylation of hemoglobin emerges as an invaluable model for the pathogenesis of certain chronic diabetes complications. In addition to reviewing a wealth of investigative possibilities in the area of these chronic complications-including eye, kidney, nerve, and vascular disease-Dr. Cohen indicates how enhanced nonenzymatic glycosylation in uncontrolled diabetes underscores the pressing need for maintenance of long-term euglycemia. Dr. Cohen is an endocrinologist and diabetes specialist whose research activities have largely focused on the chemistry and metabo lism of the basement membrane in diabetes. This superb monograph on nonenzymatic glycosylation clearly shows the major trends of her past and present research and clinical activities. This book is beautifully written and a pleasure to read. It provides great insight into the mechanisms of the pathogenesis of the oom- vii viii Foreword cations of diabetes and should be of immense value not only to basic and clinical investigators, but also to internists, diabetologists, and endocrinologists in clinical practice.

Notes On Clinical Biochemistry shows how the details of molecular architecture and metabolic pathways which every biochemistry student has to learn can be summarised and made applicable to a wide range of clinical problems. This is done partly by a theoretical discussion of the links between the basic biochemistry and corresponding medical scenarios, and partly by the discussion of case histories. In this way, a very wide range of medical problems is discussed economically. This book is aimed at all those studying clinical chemistry, and medical students in the middle period of their course, that is the 2nd — 4th years. This is when the knowledge of basic sciences gained in the earlier period comes to be applied to clinical situations, and the book attempts to show, with respect to biochemistry, how this continuum is achieved. Why are pills not as safe and effective as insulin treatment? What is the genetic risk to an individual of developing diabetes? What can at-risk individuals do to prevent the onset of diabetes? For straightforward and informed answers to questions like these, turn to Clinical Diabetes Mellitus: A Problem Oriented Approach. The third edition of this acclaimed work represents the state-of-the-art in diabetes research, diagnosis, and management - in a unique problem-oriented format. Reflecting Dr. Davidson's more than 50 years of experience in the research and care of diabetic patients, this format offers direct solutions to the problems diabetes care professionals face everyday. The book features several new chapters and is divided into seven sections covering: the nature of diabetes mellitus; screening and diagnosis; long-term management; complications; concomitant problems; socioeconomic problems; and the development and evaluation of diabetes care programs. Inside, you'll find insightful analysis of epidemiology, pathophysiology, patient adherence, the latest research, and more. Plus, the book provides state-of-the-art information on everything from new insulin analogues and treatment methods to diabetes education and the significance of the Diabetes Control and Complications Trial. Highlights of the third edition: New chapters on cranial and peripheral neuropathy, autonomic neuropathy, and musculoskeletal problems in diabetes Comprehensive chapters on the team approach to diagnosis and treatment of diabetes with detailed information on how to deal with psychosocial problems New recommendations of the American Diabetes Association for the diagnosis of Type 1, Type 2, and gestational diabetes mellitus Thorough presentations on all diabetes related complications, including retinopathy, nephropathy, heart disease, cerebrovascular disease, and the diabetic foot Helpful information on the public health responses of the CDC and diabetes research programs of the NIDDK and NIH Contributions from 106 world-class authors from the U.S., Canada, and Europe For comprehensive, up-to-date, and clinically relevant information on diabetes mellitus, this book is unparalleled.
