The 7th edition comes 32 years after the publication of the first Study Guide by American College of Emergency Physicians in 1978! The textbook changes over the years have mirrored the complexity and expansion of the practice of emergency medicine. Emergency physicians are specialists in time-focused treatments such as trauma, stroke, shock, and heart attack, and the 7th edition provides the most contemporary information as possible for acute conditions. Emergency Medicine is the only population-based specialty and is the only specialty that serves as the safety net for those with illnesses, injuries, socioeconomic barriers to care, and conditions that require the mobilization of resources for care on a 24/7 basis. We have expanded the scope of the book to represent all those conditions for which patients seek ED care, and to include recommendations for transitions of care to inpatient and outpatient services. We have tried to construct the book so it is a primary resource for ED conditions from A to Z—well, almost A to Z—Air Transport to Xanthines. As a primary resource, the 7th edition has expanded procedure content and provides a series of procedure videos to accompany this text. The full collection of videos is available on AccessEmergencyMedicine.com, and the collection will continue to expand. The video collection with this text is aimed at both learner and teacher, to learn and teach the precise steps and potential errors with each procedure. Emergency Medicine is a global specialty and is officially recognized as a specialty in nearly 50 countries around the world. The problems, challenges, and practices of emergency are globally similar. The 7th edition is notable for contributions by emergency physicians from 17 countries. The editors of the 7th edition are all actively practicing emergency physicians, and their guidance, insight, and careful attention have been invaluable in its preparation. Thank you Steve, John, David, Rita, and Garth. Barbara Steckler, BS, EMT-P, has served as our managing editor in Chapel Hill and has been invaluable in managing the manuscripts for us. Many of our medical illustrations have been drawn by Cinnamon Larson, PhD. We have been fortunate to work with Anne Sydor, PhD, as our McGraw-Hill Medical Editor. Anne has special gifts in project organization and management, as well as an in-depth knowledge of medical science. Anne’s commitment to this project has been exemplary, and I can honestly say that her energy and perseverance have enabled us to produce the best edition ever. Judith E. Tintinalli, MD, MS Editor-in-Chief Chapel Hill, North Carolina

We have produced this first edition of The 5-Minute Pediatric Emergency Medicine Consult with the specific goal of providing a concise, formatted resource for the clinical management of pediatric emergencies and common pediatric complaints encountered in emergency departments and other settings. As academic pediatric emergency physicians, our goal is to apply the best evidence, published clinical practice guidelines, and other information to optimize patient care. Numerous constraints may interfere with one’s ability to stay appraised of the most current evidence and guidelines applicable to pediatric emergency medicine. We recognize this and present this book as a resource for those who also wish to practice evidence-based medicine but require the information distilled in a single, easily accessible reference. This 5-Minute Pediatric Emergency Medicine Consult is exactly such a reference book. Honorifically named for the Fleisher and Ludwig Textbook of Pediatric Emergency Medicine, which is the seminal comprehensive textbook of pediatric emergency medicine, our text seeks to provide a rapid reference for use in the clinical workplace rather than thorough, extensive coverage of the subject matter. The 5-Minute Pediatric Emergency Medicine Consult is intended to be accurate, pointed, and readily integrated into practice rather than definitive. Clinicians at all levels of training and all stages of career development need references to facilitate the provision of optimal patient management. Each chapter presents information in a thorough but very concise, succinct manner so that all key points about clinical presentations, differential diagnoses, diagnostic investigations, therapeutic interventions including medications and doses, and admission or discharge criteria are presented. This text has been authored by selected pediatric emergency physicians from both emergency and pediatric medicine backgrounds to provide true subspecialty expertise. The 5-Minute Pediatric Emergency Medicine Consult will be useful to trainees as well as to clinicians far beyond training. Though the editors and authors practice pediatric emergency medicine within the confines of emergency departments in academic medical centers, we recognize that most pediatric emergencies are not treated in such settings, and many pediatric emergencies are treated outside of an emergency department altogether. Thus, we have taken great strides to produce a reference that represents optimal management of emergent and urgent pediatric conditions as well as common pediatric complaints in all such locations. Clinicians practicing in a tertiary care academic medical center with extensive subspecialty support or those in the community setting in which little or no subspecialty support will find this text to be an invaluable resource. We hope this book will serve as a useful and frequently used resource to assist in our common goal of providing the best possible pediatric emergency care to our patients. Five Minutes – What can you do in 5 minutes? Working in the pediatric emergency department for more than 35 years has often been a humbling experience. Along the way, we have learned many lessons. One of the most important has been the value of orientation and preparation. We have all provided care for children when they suddenly burst through the doors in the arms of a police officer or paramedic with major multisystem trauma or cardiac arrest. We have been in the same critical situation when we had just 5 minutes to prepare for the event. As many of the readers of this book know, that extra 5 minutes makes a world of difference to the outcome and perceived quality of care provided. Even when residents are assigned to the emergency department for a rotation it is our perception that those who get a brief orientation to the site perform much better than those who do not. And those who take a minute to prepare before going into a treatment room also evolve to be better physicians. That brings us to this book, The 5-Minute Pediatric Emergency Medicine Consult. We know that using this book will provide physicians with the orientation and preparation needed to address a wide range of clinical problems. Sometimes it will be a new condition that the physician has not faced before, and the topic will give an important overview. At other points, it will be used for a familiar or common clinical situation but a 5-minute refresher will provide a reassuring double check to make sure nothing has been overlooked or forgotten. In either case, that initial orientation and preparation will prove most valuable. We are honored to have our names as part of the title of this book. We view it as a perfect companion to the Textbook of Pediatric Emergency Medicine, 6th Edition where more in-depth information can be learned. We thank the editors and authors for allowing us to share in this work. We see it as yet another important step in improving the emergency care to ill and injured children, which has been the foundation of our life’s work. Five minutes can mean a lot, and 5 minutes of reading one of the topics that follow will set you on a pathway toward rewarding clinical care. STEPHEN LUDWIG, MD Philadelphia, Pennsylvania GARY R.
